I hereby give my permission for images of my child, captured during regular and special CYO Basketball
activities through video, photo, or digital camera, to be used solely for the purposes of St. Mary’s CYO
Basketball promotional materials, publications, and team photo galleries, and waive any rights of
compensation or ownership thereto. I further agree that these images of my child may be posted on various
internet websites for photo sharing, our parish, league, and/or the archdiocese.
